The Green Bay Packers currently are accepting nominations for the Community Quarterback Award, presented by Humana Inc.

Funded by the Green Bay Packers, the NFL Foundation and Humana, the Community Quarterback Award recognizes outstanding volunteers within the state of Wisconsin. Non-profit 501(c)(3) organizations are invited to nominate exceptional volunteers who demonstrate leadership, dedication and a commitment to bettering their communities through volunteerism.

The Community Quarterback Award is for volunteers who are not compensated for their services and have served a minimum of one year with their organization. Recipients will be selected from the following categories: volunteer leadership (for example, board president or board member), youth (under the age of 18), adult (ages 18 to 65) and senior (over the age of 65). A total of 10 recipients will be selected.

For the fourth year, Humana will be matching all funds received from the NFL Foundation, raising the total amount awarded to $40,000, with each volunteer receiving $4,000 for his or her respective charity. In addition to the grants, each winner will also receive two tickets to a 2015 Packers home game, provided by Humana.

“Our goal is to make the communities we serve 20 percent healthier by the year 2020, and a big part of that is giving back,” said Tami Quiram, president of Humana’s commercial division. “As the largest employer in Green Bay, Humana now offers our own employees paid volunteer time off to ensure we all take the time to support others and promote community well-being.”

Applications now are available online at www.packers.com/CQB. The deadline for nomination submission is Monday, Sept. 28, 2015.

Further information can be obtained through the Packers’ community outreach department at 920/569-7500 or via e-mail at IGAMFoundation@packers.com.

The Community Quarterback Award is part of Green Bay Packers Give Back, the Packers’ all-encompassing community outreach initiative. Combined with other Green Bay Packers charity endeavors, the awards contribute to a comprehensive Packers charity impact that was in excess of $7 million in the past year.
